MEMBERSHIP APPLICATION DUE JULY EACH YEAR
Don't forget - your annual membership fee of $25 is due by June 30th.
To participate in any club activities, you must be a member of the Life Activities Club Surfcoast inc.
Membership application forms are available from the membership secretary, from this web site or from Spring Creek Community House.
The current annual membership fee is $25 with a joining fee of $5 per person. (Pro-rata fee, effective Jan 1st - $12.50)
MEMBERSHIP APPLICATION INSTRUCTIONS
1. Copy membership application below and paste into word, then print.
2. Post completed form to the Treasurer, Life Activities Surfcoast Inc., PO Box 242 Torquay Vic 3228 with a cheque/money order payable to Life Activities Club Surfcoast inc.
3. Receipt of your Membership Card(s) is confirmation of acceptance into the Club. It should be carried when attending any Club activities.
4. Membership subscription covers the cost of running the Club (newsletters, postage, membership card, insurances) and entitles you to join any of the activity groups organized by the Club (subject to vacancy) as well as attend Members meetings. Activity groups set their own charges.
